I won some awards from middle school including math counts (a mathematical contest) and national history day regionals...should I include them on the college application or are they worthless? I think the answer is no, but I heard that some people included the awards they earned in middle school.

Oh by the way, I have another question...I attend a religous school for three hours each week...is this an activity?

if u put up middle school awards that kind of looks bad, because its saying u haven't really done anything/done so little in highschool that u need to count them. I think the relgious school is an ec

I agree that the religious school is an ec.
The general rule is no middle school awards, but if you were, say, top-10 at nationals in Mathcounts I think I would go ahead and include it. If it was only a chapter-level Mathcounts award, I wouldn't. And, of course, if you win a high school contest while you are still in middle school, you should include that (like the 3-4 middle schoolers every year who qualify for the USA Math Olympiad).
