1) is the "academic honors and programs" section of the app a place to put down all my internal or external awards? or both?
2) i have a cousin who went to Stanford approx. 10 years ago, but i don't know the exact year he graduated and what he majored in (and i won't have a chance to ask him). should i put his name down on the app anyway? would it have a negative effect on me if i got his graduation year wrong?

You can put "not sure" for graduation year, or something like that.
As for the awards - put whatever fits in the space, starting with the most significant ones (national or state level).
