I've heard of it, but I'm confused about the process. How does it work? I've heard of students who don't do well in high school go to college and transfer, how is this possible?

Let's just say that if you didn't get in to ivy league right after high school, you will have more chances of being struck by a lighting on your way to community college than transferring to Harvard. Then again, anyone and everyone is welcome to try anything and everything. Whatever.
