Make sure that you are not sleep-deprived going into the test - this means getting enough sleep for at least 3 nights before the test - this is probably the hardest part with your hs schedule.
Eat a good breakfast - not a heavy one that will make you feel sluggish. Orange juice and a banana will provide the electrolytes you need. A bagel and lox or the equivalent will keep you going. I have seen some kids post that they take a power bar or some such for the test break.
If you don't take gym, adding a little light exercise (a walk or jog) to your routine next will energize you a bit. Getting up early enough to take a short walk and shower will help ensure that your body is awake for the test.

just focus on focusing. When your mind begins to wander, tell yourself to focus, will power can do it.

Immerse yourself in the test. It's like a little world where you solve riddles to get to the next stage. I never have a problem with concentration because I'm used to focusing intently in sports and such. Sometimes I think of the test as a fight, like a boxing match between me and the test. There are a few breaks here and there to get it together and strategize, then I go back in for some more. Sports definitely do help with academics.

I thought everything went perfectly when I took my Calc AB exam last May.
The previous night I did some tai chi with my dad. It really worked for me, as I fell asleep immediately.
6:45PM came and the alarm went off. I just shot out of bed and did my personal hygiene routine.
I ate my usual cold cereal and orange juice breakfast. Except, I had some cheese. Eat that, you won't feel hungry.
My teacher was nice enough to buy us some sausage biscuits and some juice.
The point is:
a) Focus!
b) Get good sleep for several days in a row.
c) Stay calm!
d) Eat a good breakfast.
e) Just do it.

God no, not caffeine pills!!! They'll make you jittery. Especially if you don't already drink things with a lot of caffeine.
Eat a good breakfast - higher on proteins, fewer on carbs except bananas, which have good carbs. Just don't change anything major that morning. I tried the high-protein thing for the first time on test day and wasn't used to it and got sick!

eh? man, you need to be more nervous ;).
I'm kidding. My little sis has that problem, and apparently, it helps if you get your hands involved. Like, use your finger to follow along the questions. sister gives self congratulatory nod after every page. Or make small gestures in the air with math problems.
and sit upright. That always works for me.
