I recieved an email from Berkeley asking for teacher recs. There are also a lot of questions i have to fill out. It also said that "failure to submit the questionnaire may jeopardize your admission consideration." did anyone else recieve this? What could this mean?

I got a Disabled Students Form I had to fill out because I missed a couple of months of school during my Junior year due to medical problems. It basically had more questions asking about what happened, how did it affect you, etc. My friend who also had medical issues got this plus the pre-questionarre thing. Though I may be speculating, I think the pre-ques. thing is for more borderline students. My friend didn't have the best stats. But I don't know...

I agree with the person above. I think the questionnaire is for borderline students. This is a snippet of the email I received:
Thank you for applying to UC Berkeley for the fall semester 2003. We would
like to gather more information about your academic and personal circumstances
before we make a decision on your freshman application.
Notice the latter part of the paragraph. "before we make a decision"... and refer to what the first person put:
"failure to submit the questionnaire may jeopardize your admission consideration."
These things can be indications that you're very borderline. If you don't submit the questions, it could put your admission consideration in "jeopardy", because your statistics may not be good enough ALONE to get you into the school. (This is the case for me.) It makes sense... On the questionnaire, they ask questions regarding your academic record based on the resources provided to you (such as school, personal reasons, etc). It may also be because I checked the equal opportunity program option. Did you guys do this?
At any rate, I don't know if I should be glad or discouraged... hmmm..
