Be concise, polite, and get to the point.
"Dear SchoolX Admissions office,
I would like to offer the following updated information in support of my application.
(Blah blah blah. Tip: Bullet form is easy on the eyes.)
Please include this letter in my admissions file.
Sincerely,
Your Name"
