my math teacher from last year recently reloacted to another state. how do i go above having him writing and sending a letter of recommendation?
do i have him send it back to me or does he send it to the colleges himself?

First, ask him (via e-mail or phone) if he could write your recommendation letters. If he says that it's ok, then mail him a letter explaining why he should write one, and give him all the application deadlines for the colleges you're applying to. Also, mail a resume, a copy of the recommendation forms, and addressed, stamped envelopes. He'll send it himself.
If you want to, you can ask for the letters before he sends them, but then you have to waive the confidentiality statement, and AdComs really don't like that.
