At my school, each student fills out a little questionare about ourselves, then give it to someone so they can write a recommendation, and that goes into our file, and it is added to every application that is sent out.....

If I've already gotten a few recommendations from teachers/guidance couselors, do I now have to go back and give them a Teacher Recommendation/Evaluation Form to fill out for each particular school that has one? Or will most scools accept a recommendation that isn't written on their particular form?
